Important Notes on Completing the Online Application
When completing the family history section please only go
back as far as the expectant parents grandparents but do
include the following: the expectant mother or father
themselves, previous children, parents, grandparents, great uncles and aunts, brothers and sisters, uncles and aunts,
cousins, nieces and nephews.
Instances where a miscarriage has occurred during a multiple pregnancy or any children which did not survive should be
included.
You should provide all relevant facts which are likely to influence whether the Insurers will accept your application and on what terms and conditions. If you do not give this information insurers may have the right to refuse any claims or to make the insurance void.
